How? She's not restrained to that spot, she can just leave. Once her friend hangs, at least. Hell, even if there's something holding her there she can just slump to the side or something.

In fact it would likely be more optimal for her to stand up and let the other girl sit on her neck/shoulders. Even if she didn't go with the obvious solution of easing the top girl into the nose so she doesn't die instantly by accidentally breaking her neck, then cut her arm bindings with the blade and get the other one down.

I assume arms and legs are tied, otherwise there would be no reason to put her hands behind her back like that, but otherwise yes the spike is kind of weird.

In the full image set, bottom girl gets injected with something and then slumps forward - sleep drug I presume. Kinda cheating out of the whole peril endurance test thing but whatcha gonna do.
