That looks like only ~4 feet of height (or bout 1.4 meters), meaning it is less than .75 psi. The key thing that determines pressure is height of the water, not volume. So if her large intestine fills up, the water will overflow the gutter instead.

Hope there is a filter though, or all the gunk that is in the gutter will be flowing into her

The woman on the inside should have put a diverter setup in the enema line so rainwater comes out of the diverter, instead of overflowing the gutters and potentially causing water damage to the fascia, and later the eaves and soffits (fascia = the vertical part that the gutter is attached to)

Now later on if the enema line is still attached when the girl is lowered, the water height will increase leading to more pressure inside her
