They're suppose to be their kids. Its really not a good depiction. The artist should have made Touma and Mikoto older.

yea im pretty sure thats his cousin who was barely shown because most of the time she looked like Misaka along with making his mom look like index. she was shown in a old photo and in concept art but this might just be the artists own interpretation of the character toarumajutsunoindex.wikia.com/wiki/Tatsugami_Otohime
