In japan he is called Green. The name change only occured for us the western market because we got the editions Red&Blue and not like the japanese Red&Green first. Green sold poorly in Japan due the poor design of the Pokemons for this edition.
Blue was made with better pokemon design, which had a more realistic appearance regarding real animals. Those design were carried over in the western release for the Edition Red.

Originally it was planned from the very beginning to add a female player character into the games.

However due some technical limitations, which Gamefreak couldn't work around the time back, she couldn't be made into a player character. That changed with the remakes though and was renamed Leaf. However she is and will always be Blue.
