The drugs are to keep him going, as well as to keep him semi-unconscious. As nice as the sex may have been, he is being forced to have sex with almost every girl in the village, and he knows that as soon as they finish, he's dead.

Anyways my idea was, that he is a warrior who came back from battle exhausted and wounded and these beautiful women are the comfort officers of this tribe/army. The sad faces could say that they are feeling for him and want to make him feel better. And the green stuff is a mixture against the pain and to make the wounds heal faster.
