The penis becomes hard as a result of an increase in blood flow to it. This most often occurs in response to stimuli of a sexual nature, causing a state known as arousal. The image above contains multiple elements which can be identified as sexual in nature, such as nudity, including exposed genitalia, and penetration of the anus. Ultimately, it is reasonable to conclude that the sexual nature of the image caused arousal, which in turn caused the penis to harden.
