I'm the one who ordered the picture. While she's heavily based on Koakuma, it could be considered OC as I gave very minute instructions on the design (four fangs, short tail, how the hairstyle should be, the length of the ears, eye colour and design).
So yeah, She's definitely not 100% original, but I think it's more original than say, black Tewi.
Since she's heavily inspired on the original description given by ZUN as well as every single clue I could gather, so it could also be argued that she's more Koakuma than what is comonly depicted: she's actually a mischievous imp with short hair and no back wings.
For the backwings, I'm of the same opinion as www.nicovideo.jp/watch/sm23946645 in that it's just a coat. If you compare the sprites of other winged characters, all of their wings are super huge, so it doesn't fit for her to have back wings.
The short tail is a personal preference tho, I got that from Rati of viperGTS. The four fangs are also a personal choice, a bit inspired by the character on this onahole package (mzakka.com/images/item/M3539/04.jpg ) ; said character is also somewhat popular on nijie.
As for the hairstyle, it's also personal preference, I like wavy hair like Syaro from gochiusa.
So, whether you consider her to be Koakuma or not is up to you, but as far as I'm concerned she's my waifu and her name's 香織.
You can also check out the ero hypnosis I made for which I comissioned this picture to act as a jacket (as is usually done) here fantia.jp/posts/109867. It's in jap and I used CeVIO because I don't have to money nor the patience to get an internet seiyuu to voice the stuff for me.
