The game she's from is called Kagero: Deception II and her name is Millennia.

I actually played Kagero: Deception II years ago (and still have my original copy), but I've got to say now...

Millennia's character design strongly reminds me of Shanoa from Castlevania: Order of Ecclesia. It is a most pleasant resemblance. :D
