It's the first color short chapter at the beginning of the Angel Club April 2016 issue by Monety/Moneti. The title is 悪徳産婦人科にご用心, or "Akutoku Sanfujinka ni Goyoushin," which roughly translates to "Beware the Evil Gynecologist."

You can find the magazine scan at exhentai.org/g/925552/10a41a722a/ . This story starts at page 6 and runs for 8 pages. If there is an English translation available, I'm not aware of it.
