It is real, this happens of Makoto chooses to abandon Sekai for Kotonoha, she ends up dating Taisuke and this scene is one of the paths where Makoto intervenes after having lunch on the roof with kotonoha. Taisuke offers to hav a threesome with Sekai to "show off" how much he sexually trains her.
