I'm going to say that Koakuma is the Red text as she is the only one I remember that refers to Patchouli as "Patchouli-sama." Plus Red Text wouldn't be used for Sakuya (only OTHER person who refers to Patchouli with -sama at the end).
Translation (my own):

"What kind of book was it?"
"Well after I got Patchouli pregnant and she gave birth to our child, we decided to have sex with said child; that kind of book."
"That's...wow..."

The context makes me agree with Anon3 that this is some sort of fantasy. I assume a doujinshi for the sake of the joke.
