Because he is obviously groping her for the sake of satisfying a Perverse Sexual Desire he has for his sister, it doesn't matter if it's her breasts, lips, hips feet, hair, etc.

Unironically, the brother doesn't actually have any sexual interest at all in his sisters despite only doing perverted stuff against them as well as ogling only them. If I recall correctly, at the end of the manga it is revealed that he only did these things for some dumb reason.

Also, this being Mary Sue Medaka, she always knew that this was the case which is why she always disliked her brother. See she never had ANY problem at all with being ogled or even being groped by her brother... what made her dislike his attention and get disgusted was because she knew it was all a facade and he was faking it.
