I'm confused at the last chapter. If she was the "first Kenpachi" does this mean she was the first captain of division 11? And why was she called Yachiru Unohana? Is that her real name? And why did she look so pissed?

She was the first 'Kenpachi' so we can assume 'yes', she was the very first captain of 11th Div.
Her real name is Yachiru Unohana but she calls herself 'Retsu' = 'Violent' to distance herself away from what she was before and to not attract others from storming 4th Div to challenge her. And now, she has to 'relapse' back to how she was as 'Kenpachi' and she might not like it. Or that could just be her Happy Face.

This is one of the few tings Kubo actually managed to correctly elude to for years. I mean, whenever she seemed to get serious, or opened her eyes, people were terrified of her.

I always knew she was somebody not to fuck with, and this just confirmed it.

This could explain why she was the only captain Aizen himself DID NOT want to fight with, if you remember correctly he said that to her when she discovered that something was not right with his "dead body". It's in the manga and it's in the anime, go and check for yourselves and you'll see....

Now I understand Kubo's words in that interview a few years ago....."Unohana Taichou will have only ONE SINGLE fight in the whole series, but that will be a fight EVERYONE will remember, because she will surprise you all"......She is the most scary Taichou if someone manage to really get her mad. That's not Aizen, that's supposedly a young Kenpachi. Since Kenpachi named Yachiru ''after a person he admired the most'' it's more likely that he met her when he was still young and developed an admiration towards her instead of being lovers like some suggest.
