The one doing the fucking is the sister, a hermaphrodite, and the one being fucked is the brother, a crossdressing trap. As far as I can garner from the Pixiv entry, they are supposed to be twins, presumably fraternal. And it's fairly common for a lot of artists to draw traps as though they had small breasts, presumably to go along with the unrealistically feminine hips, unrealistically feminine ass, and unrealistically feminine waist many of them are portrayed as having.

Hormones cause most of these changes. Even in a fully developed male, long-term hormone therapy will still cause these changes, as most of the bones are situated by fat and other tissues, so as those tissues are rebuit, relocated and reorganized, the apparent shape of the skeletal structure changes. And of course... breasts grow in.

While that is true, honestly, who really cares if there's a decent explanation behind it? Most art is certainly not photorealistic, and has their own forms of stylization, all of which are in their own ways 'unrealistic'...hormone pills could be an explanation, yet in many doujin and other such, there are often no such explanations provided, the trap simply just looks very much like a female normally (though there certainly are a share of them where hormones, plastic surgery, and even brainwashing are liberally applied)...ultimately, many artists just want to draw what is technically a male that looks (and often acts) very much like a female, and this can easily include the existence of what looks like small or rudimentary breasts. Am I complaining about this lack of realism? Certainly not. I've even seen pictures marked as 'shota' on Pixiv with quite sizeable breasts, oddly enough. Or who have multiple penises. Or are pregnant. Or are part plant. And you know what? I'm all right with that.
