Because Saturn appears in Sailor Moon Super S. Idem for Neptun and Uranus.

But the picture implies Saturn is an outer senshi like Pluto, Neptune, and Uranus, when they(Pluto, Uransu and Neptune) have only the vaguest idea who Saturn is, and besides Saturn is the Grail. I feel like I'm the only person to have read and watched the series sometimes.

I saw the whole anime but I didn't read the manga. From what I understand of this series and Sailor Stars, Saturn IS an outer senshi. Since this is unofficial fan-art, it really doesn't matter.

There's also the fact that they all live together post-S. Whether she's one of the "original" Outers or not, they stick together.
