It's based on a meme from 2chan. I think the jap McDonald's were giving out Naruto toys and amongst them were these hideous Sasuke toys. So it was posted on 2ch and it became a meme. I think that's how it goes.

It indeed seems to refer to that Sasuke toy as the face is a copy of the toy and the toy became famous as it seems you can balance any type of object on his head, there are may photos proving so it's quit hilarious.
