To elaborate, Its the Original Ocarina of Time Mirror Shield obtained in the Spirit Temple.
The Original (early N64 edition) had a moon and star motif as did the Gerudo symbol, but was subsequently changed to the weird box thingy on later N64 / GC / 3DS releases because the people at Nintendo thought it might offend certain faiths due to its close approximation of a holy symbol. Same with the original chanting in the Fire Temple. Do note however no complaints against either shield or song were ever issued. It was a choice Nintendo made without outside influence.. so to speak.
A shame really, the moon and stars mirror shield was the best looking shield hands down.
